ABSTRACT

Objective: To analyze the status of excess heart age and its risk factors among Chinese residents aged 35 to 64 years. Methods: The study subjects were Chinese residents aged 35 to 64 years who completed the heart age assessment by WeChat official account "Heart Strengthening Action" through the internet from January 2018 to April 2021. Information such as age, gender, body mass index (BMI), blood pressure, total cholesterol (TC), smoking history, and diabetes history was collected. The heart age and excess heart age were calculated according to the characteristics of individual cardiovascular risk factors and the heart aging was defined as excess heart age≥5 years and 10 years respectively. The heart age and standardization rate were calculated respectively based on the population standardization of the 7th census in 2021.CA trend test was used to analyze the changing trend of excess heart age rate and population attributable risk (PAR) was used to calculate the contribution of risk factors. Results: The mean age of 429 047 subjects was (49.25±8.66) years. The male accounted for 51.17% (219 558/429 047) and the excess heart age was 7.00 (0.00, 11.00) years. The excess heart age rate defined by excess heart age≥5 years and ≥10 years was 57.02% (the standardized rate was 56.83%) and 38.02% (the standardized rate was 37.88%) respectively. With the increase of the age and number of risk factors, the excess heart age rate of the two definitions showed an upward trend according to the result of the trend test analysis (P<0.001). The top two risk factors of the PAR for excess heart age were overweight or obese and smoking. Among them, the male was smoking and overweight or obese, while the female was overweight or obese and having hypercholesterolemia. Conclusion: The excess heart age rate is high in Chinese residents aged 35 to 64 years and the contribution of overweight or obese, smoking and having hypercholesterolemia ranks high.

ABSTRACT Objective: To estimate the prevalence of overweight among Brazilian adults aged 20 to 59, according to sociodemographic characteristics, health-related behaviors, and food consumption. Methods: A cross-sectional study based on data from a population-based survey in a major metropolitan city in the state of São Paulo, Brazil, conducted between 2015-2016. Prevalences and prevalence ratios were estimated using Poisson regression; food consumption means were estimated using linear regression. Results: We analyzed data from 855 adults, 61% of whom were overweight. The prevalence of overweight was significantly higher among males, those aged 30 or older, with 8 to 11 years of education, and those who reported eating more than they should. The body mass index was significantly associated with hypertension, diabetes, high cholesterol, waist-to-height ratio, taking weight-loss medications, overeating, and the habit of checking labels. Overweight adults reported eating meat with visible fat and drinking soda more frequently than those not overweight. Overweight adults reported eating significantly more grams of food daily and had a higher intake of energy, total fat, saturated fats, trans fats, carbohydrates, protein, insoluble dietary fiber, sodium, and potassium. Their diets had a higher glycemic load when compared to participants who were not overweight. Conclusion: Adults with and without overweight differed in their sociodemographic, dietary, and clinical characteristics. Diet quality was similar between both groups, suggesting a need for improving dietary habits in this population regardless of body weight.


RESUMO Objetivo: Estimar a prevalência de excesso de peso entre adultos brasileiros, de 20 a 59 anos, segundo características sociodemográficas, de comportamentos relacionados à saúde e quanto ao consumo alimentar. Métodos: Estudo transversal, de pesquisa de base populacional, em uma cidade metropolitana de São Paulo - Brasil, conduzida entre os anos de 2015 e 2016. Foram estimadas as prevalências e as razões de prevalência por meio da regressão de Poisson, e as médias de consumo alimentar pelo uso da regressão linear. Resultados: Foram analisados dados referentes à 855 adultos, 61% destes apresentavam sobrepeso. A prevalência de excesso de peso foi significativamente maior entre: homens, com 30 anos ou mais, pessoas que possuíam entre 8 e 11 anos de estudo e entre aqueles que acreditavam comer mais do que deveriam. O índice de massa corporal foi significativamente associado à hipertensão, diabetes, colesterol alto, razão cintura-estatura, uso de medicamentos, comer mais do que deveria e o hábito de checar rótulos. Adultos com excesso de peso ingeriam carnes com gordura e refrigerantes em maior frequência quando comparados à adultos com peso saudável. Adultos com excesso de peso consumiam, significativamente, mais gramas de alimentos por dia e apresentaram maior ingestão de calorias, gorduras totais, saturadas e trans, carboidratos, proteína, fibras insolúveis, sódio e potássio. A dieta deles continha uma maior carga glicêmica quando comparada àqueles com peso saudável. Conclusão: Adultos com e sem excesso de peso, diferiram quanto às características sociodemográficas, dietéticas e clínicas. A qualidade da dieta foi similar em ambos os grupos, o que sugere a necessidade de melhora dos hábitos alimentares da população, independentemente do peso corporal.

RESUMO Objetivo: estimar a prevalência de dislipidemias e os fatores associados em adultos de Rio Branco, Acre. Métodos: estudo de delineamento seccional, populacional, que avaliou adultos (18 a 59 anos) residentes nas zonas urbana e rural de Rio Branco em 2014. As dislipidemias foram definidas conforme os critérios da Atualização da Diretriz Brasileira de Dislipidemias e Prevenção da Ateroscle rose. Empregou-se regressão logística para estimar as odds ratio (OR) e intervalos de confiança de 95% (IC95%). Resultados: a prevalência de dislipidemia, HDL-c baixo, hipertrigliceridemia isolada, hipercolesterolemia isolada e hiperlipidemia mista foi de 56,1%, 37,4%, 23,6%, 9,8% e 3,5%, respectivamente. No modelo final multivariado, apenas a obesidade (OR = 1,86; IC95%: 1,12;3,10) manteve associação estatisticamente significativa com a dislipidemia. Entre os subtipos de dislipidemias, associaram-se à hipertrigliceridemia isolada as variáveis: faixa etária de 40 a 49 anos (OR = 2,17; IC95%: 1,53;4,80); hipercolesterolemia isolada (OR = 2,52; IC95%: 1,23;5,15); HDL-c baixo (OR = 2,53; IC95%: 1,65;3,86); obesidade (OR = 2,10; IC95%: 1,25;3,53); e diabetes mellitus (OR = 5,41; IC95%: 1,46;20,4). Conclusão: a prevalência de alterações lipídicas foi elevada entre adultos. Estratégias de intervenções para diagnóstico, tratamento e intensificação de medidas preventivas e orientações de estilo de vida saudáveis são importantes nessa população.

Las dislipidemias fueron definidas conforme a los criterios de la Actualización de la Directriz Brasileña de Dislipidemias y Prevención de la Aterosclerosis. ABSTRACT Objective: to estimate the prevalence of dyslipidemia and associated factors in adults in Rio Branco, Acre. Methods: a cross-sectional, population-based study that assessed adults (age 18 to 59 years) living in urban and rural areas of Rio Branco in 2014. Dyslipidemias were defined according to the criteria of the Brazilian Guidelines Update on Dyslipidemias and Prevention of Atherosclerosis. Logistic regression was used to estimate odds ratios (OR) and 95% confidence intervals (CI95%). Results: the prevalence of dyslipidemia, low HDL-c, isolated hypertriglyceridemia, isolated hypercholesterolemia and mixed hyperlipidemia was 56.1%, 37.4%, 23.6%, 9.8% and 3.5%, respectively. In the final multivariate model, only obesity (OR = 1.86; CI95%: 1.12; 3.10) maintained a statistically significant association with dyslipidemia. Among the dyslipidemia subtypes, the following variables were associated with isolated hypertriglyceridemia: age group 40 to 49 years (OR = 2.17; CI95%: 1.53; 4.80); isolated hypercholesterolemia (OR = 2.52; CI95%: 1.23; 5.15); low HDL-c (OR = 2.53; CI95%: 1.65; 3.86); obesity (OR = 2.10; CI95%: 1.25;3.53); and diabetes mellitus (OR = 5.41; CI95%: 1.46; 20.4). Conclusion: the prevalence of lipid alterations was high among adults. Intervention strategies for diagnosis, treatment and intensification of preventive measures and healthy lifestyle guidelines are important in this population.

OBJECTIVE@#To investigate the abnormality and distribution of plasma cholesterol levels in single-center hospitalized children.@*METHODS@#The blood lipid levels of children aged 2-18 years who had blood lipid test results in Peking University First Hospital from June 2016 to June 2019 were etrospectively analyzed. Cholesterol oxidase method was used for total cholesterol, and high-density lipoprotein cholesterol and low-density lipoprotein cholesterol were detected by clearance method. The counting data were compared with chi-square test.@*RESULTS@#The survey had involved 11 829 children (7 087 were boys and 4 742 were girls). 1 822 (15.4%) children were with elevated total cholesterol, 1 371 (11.6%) children with elevated low-density lipoprotein cholesterol, and 2 798 (23.7%) children with high-density lipoprotein cholesterol reduction. The total number of the children with abnormal cholesterol levels was 4 427 (37.4%). Among the 7 835 children who visited hospital due to the disease not commonly inducing dyslipidemia, 731 (9.3%) had elevated TC, 561 (7.2%) had elevated LDL-C, 1 886 (24.1%) had decreased HDL-C, and 2 576 (32.9%) had abnormal cholesterol levels. Among the children with different diseases, the difference in the incidence of abnormal cholesterol was statistically significant. The top three main groups of the children with increased total cholesterol and low-density lipoprotein cholesterol were "dyslipidemia", "urinary tract disease", and "nutritional disease"; The top three main groups of the children with reduced high-density lipoprotein cholesterol were "respiratory diseases", "dyslipidemia", "hematological diseases and malignant tumors". Among the 1 257 blood li-pid test results sent by other departments, 300 cases had abnormal cholesterol levels (23.8%). Among them, there were 70 children with hypercholesterolemia (5.6%), 44 children with increased low-density lipoprotein cholesterol (3.5%), and 224 children with reduced high-density lipoprotein cholesterol (17.8%). There were 365 (4.6%) children with low-density lipoprotein cholesterol ≥140 mg/dL (3.6 mmol/L) who needed to further exclude familiar hypercholesterolemia among the children who visited hospitals due to the disease not commonly inducing dyslipidemia.@*CONCLUSION@#Children in hospitals have a high incidence of cholesterol abnormalities. Doctors need to pay more attention to the cholesterol diagnosis and management regardless of the discipline, which not only helps to control secondary hypercholesterolemia, but also provides the possibility of detecting familial hypercholesterolemia in time.

ABSTRACT

RESUMEN La cardiopatía isquémica y los accidentes cerebrovasculares son la primera causa de muerte en el mundo. La enfermedad cardiovascular de origen ateroesclerótico es un problema internacional de salud, que constituye una carga social, sanitaria y económica. Se realizó un análisis de las principales guías internacionales sobre dislipoproteinemias y su manejo, como las de la Sociedad Europea de Cardiología y las del Colegio Americano de Cardiología/Asociación Americana del Corazón. También, de los principales artículos publicados en los últimos cinco años sobre el manejo de la hipercolesterolemia, de los cuales se tomaron 20 publicaciones en Medline, Google Académico y SciELO. Las mencionadas guías reúnen las recomendaciones de sus respectivas organizaciones y las combinan con nuevas. Ambas mantienen el uso de scores de riesgo y discrepan sobre la imagenología en la determinación del tratamiento, al igual que en el uso de drogas no estatinas. Se plantea que la mejor intervención para prevenir la enfermedad cardiovascular es la promoción de un estilo de vida saludable (AU).


ABSTRACT Ischemic cardiomyopathy and cerebrovascular stroke are the first causes of death in the world. Cardiovascular disease of atherosclerotic origins is an international health problem that is also a social, sanitary and economic burden. The authors analyzed the main international guidelines on dyslipoproteinemia, like the ones from the European Society of Cardiology and the American College of Cardiology/American Heart Association. They also considered the main articles published in the last five years on the management of hypercholesterolemia and chose 20 of them available in Medline, Google Scholar and SciELO. The before-mentioned guidelines gather the recommendations of their own organizations, and combine them with new ones. They both keep using risk scores on and differ on medical imaging determining the treatment, and also in the use of non-statin drugs. It is stated that the better intervention to prevent cardiovascular disease is the promotion of a healthy lifestyle (AU).

Resumo O objetivo deste artigo é comparar as prevalências autorreferidas e medidas por exames laboratoriais, assim como a ocorrência de valores de falsos positivos e negativos, para diabetes, doença renal crônica e hipercolesterolemia. Foram utilizadas informações da entrevista e exames laboratoriais da Pesquisa Nacional de Saúde (2013, 2014-2015). Foram calculadas a sensibilidade e a especificidade, segundo sexo, idade, escolaridade, ter plano de saúde e tempo desde a última consulta médica. Por meio de regressão logística, foram analisados fatores associados à ocorrência de falsos positivos e falsos negativos. A sensibilidade foi mais elevada para o diabetes e entre os idosos e os que tiveram consulta médica mais recentemente. A especificidade foi alta para todas as doenças, com melhor desempenho entre os jovens, os com alta escolaridade e os que consultaram há mais de um ano. As chances de falsos positivos e falsos negativos diminuíram com a escolaridade e aumentaram com a idade. A sensibilidade baixa indica que as prevalências podem ser mais elevadas que as medidas autoreferidas apontam.


Abstract This paper aims to compare the self-reported prevalence measured by laboratory tests and the false positive and negative values for diabetes, chronic kidney disease, and hypercholesterolemia. We used information from the interview and laboratory tests of the National Health Survey (2013, 2014-2015). Sensitivity and specificity were calculated by gender, age, schooling, having health insurance, and time since the last medical visit. We used logistic regression to analyze associated factors with false positives and negatives. Sensitivity was higher for diabetes and among older adults and those who had a medical visit more recently. Specificity was high for all diseases, with better performance among younger people, those with high schooling, and a visit more than one year ago. The likelihood of false positives and negatives decreased with schooling and increased with age. Low sensitivity suggests that prevalence might be higher than indicated by self-reported measures.

Introducción: La hipercolesterolemia está asociada a enfermedades cardiovasculares en la población adulta mayor y, además, a factores de riesgo como: edad, esperanza de vida y capacidad funcional. El envejecimiento como transformación social del siglo XXI constituye una situación epidemiológica relacionada con los factores de riesgo de las enfermedades cardiovasculares, se clasifican en modificables (hipertensión arterial, niveles elevados de colesterol, tabaquismo, obesidad) y no modificables (edad, sexo, antecedentes familiares). El tabaquismo potencia esta enfermedad, al igual que la hipertensión arterial multiplicando las limitaciones funcionales. Objetivo: Describir la hipercolesterolemia como factor de riesgo cardiovascular en adultos mayores. Métodos: Se realizó un estudio de revisión bibliográfica mediante la consulta sistemática de artículos y libros publicados en bases de datos electrónicos como: PubMed, Scielo, Dialnet, Cochrane plus, Redalyc, Elsevier, además de organismos nacionales e internacionales, durante el periodo enero-junio 2019. Para la búsqueda de información se utilizaron descriptores en salud como hipercolesterolemia, adulto mayor, factores de riesgo y boleano (and). También se consultaron páginas web como: Organization for Economic Cooperation and Development, Cardiovascular disease and diabetes, American Diabetes Association, Cardiovascular Disease and Risk Management. Conclusiones: La hipercolesterolemia constituye un factor de riesgo modificable, por tanto, se recomienda establecer estrategias de prevención que modifiquen de manera positiva el estilo de vida del adulto mayor. Además, la utilización de estatinas debe ser en dosis de baja a moderada intensidad, considerando las interacciones farmacológicas y efectos adversos, presencia de comorbilidades y expectativa de vida(AU)


Introduction: Hypercholesterolemia is associated with cardiovascular diseases in the elderly population and, also, with risk factors such as age, life expectancy, and functional capacity. Aging as a social transformation of the twenty-first century constitutes an epidemiological situation related with the risk factors for cardiovascular disease. They are classified as modifiable (high blood pressure, high cholesterol levels, smoking, obesity) and non-modifiable (age, sex, family history). Smoking potentiates this disease, as does high blood pressure by multiplying functional limitations. Objective: To describe hypercholesterolemia as a cardiovascular risk factor in older adults. Methods: A bibliographic review study was carried out through systematic consultation of articles and books published in electronic databases such as PubMed, Scielo, Dialnet, Cochrane plus, Redalyc, Elsevier, in addition to national and international organizations, during the period from January to June 2019. In order to search for the information, health descriptors were used, such as hipercolesterolemia [hypercholesterolemia], adulto mayor [older adults], factores de riesgo [risk factors], together with the Boolean operator and. We also consulted websites such as the Organization for Economic Cooperation and Development, Cardiovascular disease and diabetes, American Diabetes Association, Cardiovascular Disease and Risk Management. Conclusions: Hypercholesterolemia is a modifiable risk factor; therefore, it is recommended to establish prevention strategies that modify the lifestyle of the older adults positively. Furthermore, statins should be used in doses of low to moderate intensity, considering the pharmacological interactions and adverse effects, as well as presence of comorbidities and life expectancy(AU)

Resumen Introducción: En Estados Unidos se dispone de información acerca de la población mexicoamericana por el Estudio de Salud y Envejecimiento del Cerebro en Latinos Mayores (HABLE); en México se dispone de los resultados del Estudio Nacional de Salud y Envejecimiento en México (ENASEM). Objetivo: Comparar la prevalencia de factores de riesgo cardiovascular entre hombres y mujeres de HABLE y ENASEM. Método: Se analizó transversalmente la prevalencia de hipertensión, diabetes, hipercolesterolemia y obesidad abdominal en 559 participantes de HABLE y se comparó con datos de 13 663 participantes del ENASEM. La comparación se realizó mediante t de Student y chi cuadrada, según el tipo de variable. Resultados: El análisis demostró que la prevalencia de hipertensión (50 %, IC 95 % = 41.8-51.8), diabetes (35.5 %, IC 95 % = 27.6-43.8) y obesidad abdominal (59.3 %, IC 95 % = 50.5-68.1) fueron significativamente mayores en hombres del HABLE, mientras que las mujeres presentaron una prevalencia más elevada de diabetes (36.8 %, IC 95 % = 32.2-41.5) y obesidad abdominal (89.6 %, IC 95 % = 86.6-92.5). La hipercolesterolemia tuvo una prevalencia más elevada en mujeres del ENASEM (53.3 %, IC 95 % = 50.3-56.2). Conclusión: La prevalencia de factores de riesgo cardiovascular fue mayor en mexicoamericanos participantes del HABLE, que en mexicanos participantes del ENASEM.


Abstract Introduction: In the United States, information on the Mexican-American population is available through the Health and Aging Brain among Latino Elders (HABLE) study; in Mexico, the results of the Mexican Health and Aging Study (MHAS) are available. Objective: To compare the prevalence of cardiovascular risk factors between men and women of the HABLE and MHAS studies. Method: The prevalence of hypertension, diabetes, hypercholesterolemia and abdominal obesity was transversely analyzed in 559 HABLE participants and compared with data from 13,663 MHAS participants. The comparison was made using Student’s t-test and the chi-square test, according to the type of variable. Results: The analysis showed that the prevalence of hypertension (50 %, 95 % CI = 41.8-51.8), diabetes (35.5 %, 95 % CI = 27.6-43.8) and abdominal obesity (59.3 %, 95 % CI = 50.5-68.1) were significantly higher in HABLE males, whereas females had a higher prevalence of diabetes (36.8 %, 95 % CI = 32.2-41.5) and abdominal obesity (89.6 %, 95 % CI = 86.6-92.5). Hypercholesterolemia had a higher prevalence in MHAS females (53.3%, 95% CI = 50.3-56.2). Conclusion: The prevalence of cardiovascular risk factors was higher in Mexican American HABLE participants, than in Mexican MHAS participants.

ABSTRACT OBJECTIVE: To assess the prevalence of multimorbidity among Brazilian adults and its association with socioeconomic indicators. METHODS: Cross-sectional study that used data from the Pesquisa Nacional Sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM - Brazilian National Survey on Access, Use and Promotion of Rational Use of Medicines), carried out between 2013 and 2014. The definition of multimorbidity was the coexistence, in a single individual, of two or more chronic diseases, measured through a list of 14 morbidities (self-reported medical diagnosis throughout life). Economic status and educational level were the socioeconomic indicators used, being the inequalities assessed through the Slope Index of Inequality (SII) and the Concentration Index, stratified by gender. RESULTS: The study comprehended 23,329 adults (52.8% of which were women), with an average age of 37.9 years. Hypertension and high cholesterol levels were the most prevalent conditions. The prevalence of multimorbidity was of 10.9% (95%CI 10.1-11.7) representing nearly 11 million individuals in Brazil, of which 14.5% (95%CI 13.5-15.4) were women and 6.8% (95%CI 5.9-7.8) were men. The occurrence of multimorbidity was similar according to the socioeconomic indicators. In the inequality analysis, we observed absolute and relative differences in men with a higher purchasing power (SII = 3.7; 95%CI 0.3-7.0) and higher educational level (CIX = 7.1; 95%CI 0.9-14.7), respectively. CONCLUSIONS: The frequency of comorbidities in Brazilian adults is high, especially in absolute terms. We only observed socioeconomic inequalities in multimorbidities among men.


RESUMO OBJETIVO: Avaliar a prevalência de multimorbidade e a associação desta com indicadores socioeconômicos entre adultos brasileiros. MÉTODOS: Estudo transversal que utilizou dados oriundos da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil, realizada entre 2013 e 2014. Multimorbidade foi definida como a coexistência, no mesmo indivíduo, de duas ou mais doenças crônicas, e é mensurada a partir de uma lista de 14 morbidades (autorrelato de diagnóstico médico na vida). Classe econômica e escolaridade foram os indicadores socioeconômicos utilizados, sendo as desigualdades avaliadas pelo Slope Index of Inequality (SII) e pelo Concentration Index (CIX), estratificadas por sexo. RESULTADOS: O estudo considerou 23.329 mil adultos (52,8% de mulheres), com média de idade de 37,9 anos. Hipertensão e colesterol alto foram as condições mais prevalentes. A prevalência de multimorbidade foi de 10,9% (IC95% 10,1-11,7), representando, aproximadamente, 11 milhões de indivíduos no Brasil, sendo 14,5% (IC95% 13,5-15,4) entre mulheres e 6,8% (IC95% 5,9-7,8) entre homens. A ocorrência de multimorbidade foi similar segundo os indicadores socioeconômicos. Nas análises de desigualdade, observou-se diferença absoluta e relativa para homens com maior poder aquisitivo (SII = 3,7; IC95% 0,3-7,0) e maior escolaridade (CIX = 7,1; IC95% 0,9-14,7), respectivamente. CONCLUSÕES: A frequência de adultos brasileiros com multimorbidade é alta, principalmente em termos absolutos. Desigualdades socioeconômicas na multimorbidade foram observadas somente entre homens.

ABSTRACT Objective: To assess the frequency of the hypertriglyceridemic waist phenotype and its associated factors in children and adolescents with type 1 diabetes mellitus. Methods: This is an observational analytical study with individuals with type 1 diabetes mellitus, aged 5 to 18 years, of both genders, followed in a university hospital in the Brazilian Northeast. Weight, height, and waist circumference were measured, and the lipid profile and glycated hemoglobin were analyzed. The hypertriglyceridemic waist phenotype was defined by the simultaneous presence of increased waist circumference (≥90th percentile for age and gender) and elevated serum triglyceride levels (≥75 mg/dL for children and ≥90 mg/dL for adolescents). We also investigated the family history of cardiovascular diseases and diabetes, as well as sociodemographic and behavioral variables. In the statistical inference tests, the proportions were compared by Pearson's chi-square test ­and/­or Fisher's exact test, being significant p<0.05. Results: A total of 102 patients were evaluated, most of them females (54.9%) and adolescents (66.7%). The frequency of hypertriglyceridemic waist was 23.5%, which was associated with females (p=0.043), overweight (p=0.023), hypercholesterolemia (p=0.002), high LDL (p=0.001), and borderline VLDL (<0.001). Conclusions: The frequency of the hypertriglyceridemic waist phenotype was associated with females, atherogenic lipid profile, and overweight, indicating the importance of the nutritional monitoring of this population, aiming at reducing future cardiovascular diseases.


RESUMO Objetivo: Avaliar a frequência do fenótipo cintura hipertrigliceridêmica e analisar seus fatores associados em crianças e adolescentes portadores de diabetes melito tipo 1. Métodos: Trata-se de um estudo observacional analítico com indivíduos com diabetes melito tipo 1, de cinco a 18 anos de idade, de ambos os sexos, acompanhados em um hospital universitário do Nordeste brasileiro. Foram realizadas medidas de peso, altura e circunferência da cintura, além da análise do perfil lipídico e da hemoglobina glicada. O fenótipo cintura hipertrigliceridêmica foi definido pela presença simultânea da circunferência da cintura aumentada (≥percentil 90 por idade e sexo) e dos níveis séricos de triglicerídeos elevados (≥75 mg/dL para crianças e ≥90 mg/dL para adolescentes). Investigaram-se, ainda, os antecedentes familiares para doenças cardiovasculares e diabetes, e também variáveis sociodemográficas e comportamentais. Nos testes de inferência estatística, as proporções foram comparadas pelo teste do qui-quadrado de Pearson e/ou exato de Fisher, sendo significante p<0,05. Resultados: Foram avaliados 102 pacientes, com predomínio do sexo feminino (54,9%) e de adolescentes (66,7%). A frequência de cintura hipertrigliceridêmica foi de 23,5%; a qual apresentou associação com o sexo feminino (p=0,043), excesso de peso (p=0,023), hipercolesterolemia (p=0,002), LDL elevado (p=0,001) e VLDL em valores limítrofes (<0,001). Conclusões: A frequência do fenótipo cintura hipertrigliceridêmica foi associada ao sexo feminino, ao perfil lipídico aterogênico e ao excesso ponderal, evidenciando a importância do acompanhamento nutricional dessa população, visando à redução de agravos cardiovasculares futuros.

RESUMO: Objetivo: Comparar a prevalência de fatores de risco cardiovascular na população de Vitória (ES) em pesquisa autorreferida por contato telefônico (VIGITEL) ou por exames clínicos e laboratoriais realizados na Pesquisa Nacional de Saúde (PNS). Método: Os inquéritos foram realizados na população adulta de Vitória (≥18anos). No VIGITEL foram entrevistados 1996 indivíduos (homens = 38%). Na PNS foi feita visita domiciliar seguida de exames clínicos e laboratoriais em 318 indivíduos (homens = 48%) selecionados em 20setores censitários da cidade. Nos dois inquéritos, as prevalências foram ajustadas para a estrutura populacional estimada para o ano de 2013. Os dados são fornecidos como porcentagens e intervalo de confiança de 95% (IC95%). Resultados: Foram encontradas prevalências similares no VIGITEL e na PNS, respectivamente, para tabagismo (8,2%; IC95% 6,7 - 9,7% versus 10,0; IC95% 6,4 - 13,6%) e hipertensão (24,8%; IC95% 22,6- 27,0% versus 27,2%; IC95% 21,8 - 32,5%). Houve diferença estatística (p < 0,01) entre o VIGITEL e a PNS, respectivamente, para as prevalências de obesidade (16,8%; IC95% 14,1 - 18,1% versus 25,7%; IC95% 20,4- 30,9%) e colesterol elevado (≥ 200mg/dL) no sangue (20,6%; IC95% 18,6 - 22,6% versus 42,3%; IC95% 36,9- 47,7%). A prevalência de diabetes também foi maior (p < 0,05) na PNS (6,7 versus 10,7%). Conclusão: A prevalência populacional de hipertensão e tabagismo foi estimada adequadamente no VIGITEL. Isso não ocorreu com a obesidade por provável viés de informação do peso corporal no VIGITEL. Os dados mostram a necessidade de melhorar a cobertura diagnóstica das dislipidemias em vista da importância do controle desse fator de risco na prevenção primária das doenças cardiovasculares.


ABSTRACT: Objective: To compare the prevalence of cardiovascular risk factors in the adult population of Vitória, Espírito Santo, Brazil, in two surveys conducted by telephone interview (VIGITEL) or by clinic and laboratory exams during the National Health Interview Survey (NHIS). Method: Data were collected from adults (≥ 18 years). In VIGITEL, 1,996 subjects (males = 38%) were interviewed. In NHIS, home visit followed by clinical and laboratory tests was made with 318 individuals (males = 48%) selected in 20 census tracts of the city. The prevalence of risk factors was adjusted to the estimated population of the city in 2013. Data are shown as prevalence and 95% confidence interval (95%CI). Results: Similar values of prevalence were found in VIGITEL and NHIS, respectively, for smoking (8.2%; 95%CI 6.7-9.7% vs 10.0; 95%CI 6.4 - 13.6%) and hypertension (24.8%; 95%CI 22.6 - 27.0% vs 27.2%; 95%CI 21.8 - 32.5%). Statistical differences between surveys (p < 0.01) were found for diabetes (6.7%; 95%CI 5.6 - 7.9% vs 10.7%; 95%CI 7.1 - 14.5%), obesity (16.8%; 95%CI 14.1 - 18.1% vs 25.7%; 95%CI 20.4 - 30.9%) and high cholesterol (≥ 200mg/dL) (20.6%; 95%CI 18.6- 22.6% vs 42.3%; 95%CI 36.9 - 47.7%). The prevalence of diabetes was also higher (p < 0.01) in NHIS (6.7 vs 10.7%). Conclusion: Prevalence of smoking and hypertension, but not obesity, was adequately detected in VIGITEL, because there might have been information bias related to body weight during telephone interviews. Datashow the necessity to improve the diagnosis of dyslipidemias in primary care services, as the control of this risk factor is of utmost importance to prevent cardiovascular diseases.

RESUMO: Objetivo: Analisar as prevalências dos níveis de colesterol total e frações alterados na população brasileira, segundo dados bioquímicos da Pesquisa Nacional de Saúde. Métodos: Estudo descritivo, utilizando dados laboratoriais da Pesquisa Nacional de Saúde coletados entre os anos de 2014 e 2015. Foram analisados exames de colesterol total e frações e calculadas prevalências populacionais de valores alterados segundo variáveis sociodemográficas. Consideraram-se os seguintes pontos de corte: colesterol total ≥ 200mg/dL; lipoproteínas de baixa densidade (LDL) ≥ 130mg/dL e lipoproteínas de alta densidade (HDL) < 40mg/dL. Resultados: Aprevalência de colesterol total ≥ 200mg/dL na população foi de 32,7%, mais elevada em mulheres (35,1%). A prevalência de HDL alterado foi de 31,8%, sendo de 42,8% no sexo masculino e 22,0% no feminino. LDL≥ 130mg/dL foi observado em 18,6%, com prevalência mais elevada em mulheres (19,9%). População com idade de 45 anos ou mais e com baixa escolaridade apresentou maiores prevalências de colesterol com alterações. Conclusão: Valores de colesterol total e frações alterados foram frequentes na população brasileira, especialmente entre mulheres, idosos e pessoas de baixa escolaridade. Esses resultados poderão orientar as ações de controle e prevenção, como alimentação saudável, atividade física e tratamento, visando à prevenção de doenças coronarianas.


ABSTRACT: Objective: To analyze the prevalence of altered total cholesterol and fractions levels in the Brazilian population, according to biochemical data from the National Health Survey. Methods: A descriptive study, using data from the National Health Survey, collected between 2014 and 2015. Total cholesterol and fractions were analyzed and population prevalences of altered values according to socio-demographic variables were calculated. The cutoff points considered were: total cholesterol ≥ 200mg/dl; low-density lipoprotein LDL ≥ 130mg/dL and high-density lipoprotein HDL < 40mg/dL. Results: The prevalence of total cholesterol ≥200mg/dL in the population was 32.7%, and higher in women (35.1%). The prevalence of altered HDL was 31.8%, 22.0% in females and 42.8% in males. LDL ≥ 130mg/dL was found in 18.6% and was higher in women (19.9%). The population aged 45 years old and older and those with low levels of education presented a higher prevalence of altered cholesterol. Conclusion: Altered values of total cholesterol and fractions were frequent in the Brazilian population, especially among women, the elderly and people with low levels of education. These results may guide control and preventative actions such as healthy eating, physical activity and treatment, all of which aim to prevent coronary diseases.

Abstract Background: Cardiovascular diseases are the leading cause of deaths in the world, and many events could be prevented by healthy life habits. Objectives: To compare the occurrence of cardiovascular risk factors in adolescents enrolled at public schools in the city of Rio de Janeiro, including a renowned school for sport practices. Methods: Cross-sectional study, convenience sampling of 422 students enrolled at the Experimental Olympic Gymnasium (EOG) and at Figueiredo Pimentel School (FP). Using descriptive analyses, continuous variables were expressed as mean and standard deviation or median and interquartile ranges, and the Student's t-test or the chi-square test, respectively, was used for comparisons. The sports were classified according to the metabolic equivalent of task (MET) (below or above 5). Results: We included 274 students enrolled at the EOG and 148 at FP. Mean age was similar between schools -12.5 ± 1.6 years at FP and 12.6 ± 0.9 at the EOG; 65.5% of the students at FP and 43.8% of the students at the EOG were female (p < 0.01). Significant differences in the prevalence of hypertension (20% vs. 6.3%, p < 0.01) and borderline cholesterol levels (27.7% vs. 17.3%, p = 0.01) were found between FP and EOG students, respectively. Conclusion: High prevalence of hypertension, overweight/obesity and altered blood lipid profile was found in this group of adolescents. Regular sports training program combined with little influence of their eating habits outside school may contribute to a better metabolic profile and reduction in cardiovascular risk factors in students. Public health measures are also need.


Resumo Fundamento: As doenças cardiovasculares são a maior causa de mortes no mundo e muitos eventos poderiam ser evitados por meio de hábitos saudáveis de vida. Objetivos: Comparar a ocorrência de fatores de risco cardiovascular em adolescentes de escolas públicas do município do Rio de Janeiro, sendo uma delas, modelo em práticas esportivas. Métodos: Estudo transversal; amostra de conveniência de 422 escolares do Ginásio Experimental Olímpico (GEO) e da Escola Municipal Figueiredo Pimentel (FP). Foi realizada análise descritiva dos dados utilizando média e desvio-padrão (DP) ou mediana e intervalo interquartil para variáveis contínuas (testes t de Student e qui-quadrado para comparação, respectivamente). As modalidades esportivas foram classificadas de acordo com o equivalente metabólico (MET), se abaixo ou acima de 5. Resultados: duzentos e setenta e quatro alunos eram do GEO e 148 da FP. A média de idade era semelhante - 12,5 ± 1,6 na EMFP e 12,6 ± 0,9 no GEO; 65,5% dos alunos eram do sexo feminino na FP e 43,8% no GEO (p < 0,01). Da amostra geral, 40% apresentaram sobrepeso ou obesidade. Observaram-se diferenças entre a prevalência de hipertensão (20% vs. 6,3%; p < 0,01 nos alunos da FP e do GEO, respectivamente) e de níveis de colesterol total considerados limítrofes (27,7% vs. 17,3%; p = 0,01 na FP e no GEO, respectivamente). Conclusão: Hipertensão, sobrepeso/obesidade e lipidograma capilar alterado foram muito prevalentes nos adolescentes. Um programa de treinamento esportivo regular com menos interferência alimentar extraescola parece contribuir para um melhor perfil metabólico e menor risco cardiovascular entre estudantes. Ainda, medidas efetivas de saúde pública são necessárias.

O objetivo do presente estudo foi validar a autorreferência de colesterol elevado em residentes de São Paulo, Brasil, e verificar os fatores associados à sensibilidade dessa informação. Foram utilizados dados do Inquérito de Saúde de São Paulo de 2015, estudo transversal de base populacional com amostra probabilística de residentes do município. Foram incluídos 886 indivíduos com informações do questionário estruturado, dosagem de colesterol total e frações ou que referiram tratamento farmacológico do colesterol elevado. A validade da informação autorreferida de colesterol elevado foi determinada por meio da sensibilidade, especificidade, valores preditivos positivo (VPP) e negativo (VPN) e kappa, considerando as dosagens de colesterol total e LDL-c e o tratamento farmacológico como padrão-ouro. Modelos de regressão logística foram realizados a fim de investigar os fatores associados à sensibilidade dessa informação. A sensibilidade da informação de colesterol elevado tendo em conta o colesterol total como referência foi de 50,6%, a especificidade de 90,19%, VPP de 51,64%, VPN de 89,82% e kappa de 0,41. Considerando o LDL-c como padrão-ouro, a sensibilidade foi 53,52%, a especificidade 89,93%, o VPP 49,22%, o VPN 91,39% e kappa 0,43. Utilizando a dosagem de colesterol total como referência, a idade (OR = 1,69; IC95%: 1,24-2,29) e o plano de saúde (OR = 2,91; IC95%: 1,06-7,99) se associaram à sensibilidade dessa informação. Considerando a dosagem de LDL-c como padrão-ouro, a idade (OR = 1,71; IC95%: 1,13-2,57), o tabagismo (OR = 3,33; IC95%: 1,08-10,27) e o plano de saúde (OR = 3,64; IC95%: 1,10-12,08) foram associados à sensibilidade dessa informação. Os resultados indicam baixa sensibilidade e VPP da autorreferência do colesterol elevado em residentes de São Paulo.


The study aimed to validate self-report of high cholesterol in São Paulo, Brazil, and verify factors associated with this information's sensitivity. Data were used from the Health Survey of the City of São Paulo 2015, a cross-sectional population-based study with a probabilistic sample of the city's residents. The sample included 886 individuals with information from the structured questionnaire, blood measurements of total cholesterol and fractions or who reported being on medication for high cholesterol. The validity of self-reported information on high cholesterol was measured according to sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and kappa index, considering total cholesterol and LDL-c and pharmacological treatment as the gold standard. Logistic regression models were developed to investigate factors associated with the sensitivity of this information. The sensitivity of information on high cholesterol using total cholesterol as the reference was 50.6%, specificity 90.19%, PPV 51.64%, NPV 89.82%, and kappa 0.41. Taking LDL-c as the gold standard, sensitivity was 53.52%, specificity 89.93%, PPV 49.22%, NPV 91.39%, and kappa 0.43. Using total cholesterol as the reference, age (OR = 1.69; 95%CI: 1.24-2.29) and having a private health plan (OR = 2.91; 95%CI: 1.06-7.99) were associated with the information's sensitivity. With LDL-c as the gold standard, age (OR = 1.71; 95%CI: 1.13-2.57), smoking (OR = 3.33; 95%CI: 1.08-10.27), and having a private health plan (OR = 3.64; 95%CI: 1.10-12.08) were associated with the information's sensitivity. The results suggest low sensitivity and low PPV of self-reported high cholesterol in residents of São Paulo.


El objetivo de este estudio fue validar el autoinforme de colesterol elevado en residentes de São Paulo, Brasil, y verificar los factores asociados a la sensibilidad de esta información. Se utilizaron datos de la Encuesta de Salud de São Paulo 2015, estudio transversal de base poblacional con una muestra probabilística de residentes del municipio. Se incluyeron 886 individuos con información del cuestionario estructurado, cantidad de colesterol total y fracciones o que informaron de un tratamiento farmacológico por colesterol elevado. La validez de la información autoinformada de colesterol elevado fue determinada por medio de la sensibilidad, especificidad, valores predictivos positivo (VPP) y negativo (VPN) y kappa, considerando las cantidades de colesterol total y LDL-c y el tratamiento farmacológico como parámetro de referencia. Se realizaron modelos de regresión logística, a fin de investigar los factores asociados a la sensibilidad de esa información. La sensibilidad de la información de colesterol elevado, teniendo en cuenta el colesterol total como referencia, fue de un 50,6%, la especificidad de 90,19%, VPP de 51,64%, VPN de 89,82% y kappa de 0,41. Utilizando la cantidad de colesterol total como referencia, la edad (OR = 1,69; IC95%: 1,24-2,29) y el plan de salud (OR = 2,91; IC95%: 1,06-7,99) se asociaron a la sensibilidad de esa información. Considerando la cantidad de LDL-c como parámetro de referencia, la edad (OR = 1,71; IC95%: 1,13-2,57), el tabaquismo (OR = 3,33; IC95%: 1,08-10,27) y el plan de salud (OR = 3,64; IC95%: 1,10-12,08) se asociaron a la sensibilidad de esa información. Los resultados indican baja sensibilidad y VPP del autoinforme de colesterol elevado en residentes de São Paulo.

Antecedentes: la valoración del riesgo cardiovascular global es importante para establecer medidas preventivas en pacientes con alto riesgo de presentar enfermedad cardiovascular. Objetivo: estimar el riesgo cardiovascular global en mujeres diabéticas tipo 2 de edad mediana. Métodos: se realizó un estudio transversal y descriptivo con 458 mujeres en edad mediana (40 a 59 años), que ingresaron consecutivamente en el Centro de Atención al Diabético de Bayamo, Granma, desde el año 2010 a 2016. El riesgo cardiovascular global se determinó según los modelos propuestos por la Organización Mundial de la Salud para nuestro país. Resultados: el riesgo cardiovascular global de las mujeres diabéticas en edad mediana fue moderado en el 10,1 por ciento (46 pacientes), alto en el 3,1 por ciento (14 pacientes) y muy alto en el 2,4 por ciento (11 pacientes). Las mujeres posmenopáusicas exhibieron un riesgo cardiovascular global significativamente mayor que aquellas que no estaban en esta etapa (OR= 1,89; IC= 1,03-3,1; p= 0,0413). El riesgo se incrementó claramente a medida que aumentó el tiempo de ocurrencia de la menopausia (p= 0,0245). La prevalencia de hipertensión sistólica, hipercolesterolemia y hábito de fumar fue mayor en las mujeres posmenopáusicas, sin que llegara a ser estadísticamente relevante. Conclusiones: el riesgo cardiovascular global de las mujeres diabéticas tipo 2 en edad mediana, de nuestro medio, se incrementa con la menopausia y con el tiempo de ocurrencia de este evento(AU)


Background: the assessment of overall cardiovascular risk is important to establish preventive measures in patients with high risk of cardiovascular disease. Objective: to estimate the overall cardiovascular risk in type 2 diabetic middle-aged women. Methods: a transversal and descriptive study was carried out with 458 middle-aged women (40 to 59 years old) who consecutively attended the Diabetics Care Center of Bayamo, Granma from 2010 to 2016. The overall cardiovascular risk was determined according to the models proposed by the World Health Organization for our country. Results: the overall cardiovascular risk of diabetic middle-aged women was moderate in the 10.1 percent (46 patients), high in the 3.1 percent (14 patients), and very high in the 2.4 percent (11 patients). Postmenopausal women showed a significantly higher overall cardiovascular risk than those who were not in this stage (OR= 1.89, CI= 1.03-3.1, p= 0.0413). The risk clearly increased as the time of occurrence of the menopause increased (p= 0.0245). The prevalence of systolic hypertension, hypercholesterolemia and smoking habit was higher in postmenopausal women but this didn´t become statistically relevant. Conclusions: the overall cardiovascular risk of type 2 diabetic middle-aged women increases with the menopause and with the time of occurrence of this event(AU)

ABSTRACT Objective The objective of this study was to determine the prevalence of metabolic syndrome (MS) and its components among pre- and postmenopausal women, as well as the association between menopausal status and MS. Materials and methods A retrospective study was conducted at a reference cardiology outpatient clinic in a city located in Northwestern Paraná State, Brazil. A total of 958 medical records of symptomatic climacteric women evaluated between 2010 and 2014 were analyzed. The study consisted of two groups: pre- and post-menopausal women. MS was characterized according to the criteria of the National Cholesterol Education Program’s Adult Treatment Panel III – NCEP-ATP III-2005. Results MS was observed in 18.5% of the total study population; 9.4% of the premenopausal women and 22.2% of the postmenopausal women displayed MS, corresponding to a relative risk of 2.75. In addition, the frequency of MS increased with age. Regarding the components of MS, postmenopausal women were more likely to have high density lipoprotein (HDL-C) levels < 50 mg/dL; systolic blood pressure (SBP) values ≥ 130 mmHg or diastolic blood pressure (DBP) values ≥ 85 mmHg; and fasting glucose levels ≥ 100 mg/dL. Conclusion MS was more prevalent among postmenopausal women than among premenopausal women.

Introducción. La hipercolesterolemia infantil aumentó en las últimas décadas. Conocer los factores ambientales y genéticos que influyen sobre ella permitiría establecer pautas de pesquisa más adecuadas. Objetivos. Analizar si existe asociación entre factores genéticos y ambientales y la hipercolesterolemia en niños. Evaluar las cualidades predictivas de las variables que muestren asociación con la hipercolesterolemia. Material y métodos. Estudio observacional, analítico, transversal. Población: alumnos de todas las escuelas de Jovita. Edad: > 6 y < 12 años. Se midió la colesterolemia total. Mediante encuestas a los padres, se evaluó la historia clínica familiar (HCF) y el nivel socioeconómico (NSE). Se registró el peso y la talla para determinar el estado nutricional. Por medio de una encuesta al niño, se identificó el nivel de actividad física y los hábitos dietéticos. Se evaluó la asociación mediante el cálculo de OR (p < 0,05). Se efectuaron pruebas diagnósticas sobre las variables para predecir hipercolesterolemia. Resultados. Se incluyeron382 alumnos. La media de colesterolemia fue de 168 mg/dl. Un 13,4% presentaron hipercolesterolemia. El sedentarismo fue del 22,8%, y la obesidad, del 10,5%. La HCF positiva, el NSE alto-mediano y la obesidad se asociaron con hipercolesterolemia (OR 2,10; 2,10 y 2,05, respectivamente). No se encontró asociación entre actividad física e ingesta de grasas y colesterol e hipercolesterolemia. La HCF positiva y el NSE alto-mediano mostraron sensibilidad (75% y 88%) para predecir hipercolesterolemia. La hipercolesterolemia de ambos padres en relación con la hipercolesterolemia del hijo mostró un OR 9,59, sensibilidad de 73%, especificidad de 71%, valor predictivo positivo de 57% y negativo de 83%. Conclusiones. La HCF positiva, el NSE alto-mediano y la obesidad se asociaron con hipercolesterolemia en el niño. La hipercolesterolemia en ambos padres presentó una asociación con la de los niños y mostró un buen potencial como factor predictor y criterio de pesquisa.


Introduction. Pediatric hypercholesterolemia has increased over the past decades. Knowing the environmental and genetic factors that have an impact on it would allow establishing more adequate screening guidelines. Objectives. To determine if there is an association between genetic and environmental factors and hypercholesterolemia in children. To assess the predictive qualities of outcome measures associated with hypercholesterolemia. Material and methods. Observational, analytical, cross-sectional study. Population: students from all schools located in Jovita. Age: > 6 and < 12 years old. The total cholesterol level was measured. A survey was administered to parents to assess their family medical history (FMH) and socioeconomic level (SEL). Weight and height were recorded to establish nutritional status. A survey was administered to children to identify their level of physical activity and their eating habits. The association was assessed by estimating the OR value (p < 0.05). Diagnostic tests were done to establish outcome measures that predict hypercholesterolemia. Results. Three hundred and eighty-two students were included. Their mean cholesterol level was 168 mg/dL, and 13.4% had hypercholesterolemia. A sedentary lifestyle was observed in 22.8%, and obesity, in 10.5%. A positive FMH, a high/ middle SEL, and obesity were associated with hypercholesterolemia (OR: 2.10, 2.10 and 2.05, respectively). No association was found between physical activity and fat/cholesterol intake and hypercholesterolemia. A positive FMH and a high/middle SEL were sensitive enough (75% and 88%) to predict hypercholesterolemia. The presence of hypercholesterolemia inboth parents in relation to hypercholesterolemia in their child showed an OR of 9.59, a sensitivity of 73%, a specificity of 71%, a positive predictive value of 57%, and a negative predictive value of 83%. Conclusions. A positive FMH, a high/ middle SEL, and obesity were associated with hypercholesterolemia in children. The presence of hypercholesterolemia in both parents was associated with hypercholesterolemia in their child and showed itself to be a great potential predictor and screening criterion.

Introducción La grasa abdominal, especialmente la visceral, está asociada con un riesgo alto de complicaciones metabólicas. El índice cintura-estatura es usado para evaluar grasa abdominal en individuos de todas las edades. Objetivo Identificar la capacidad del índice cintura-estatura (ICE) para detectar riesgo metabólico en niños mexicanos de edad escolar. Pacientes y método Se estudiaron niños entre 6 y 12 años. Se diagnosticó obesidad con un índice de masa corporal (IMC) ≥ percentil 85, y obesidad abdominal con ICE ≥0,5. Se midieron niveles sanguíneos de glucosa, colesterol y triglicéridos. Se determinó sensibilidad, especificidad, valor predictivo positivo y negativo, área bajo la curva, coeficiente de verosimilitud positivo y negativo de la RCE e IMC para identificar alteraciones metabólicas. Se compararon ICE e IMC para determinar cuál tiene mejor eficiencia diagnóstica. Resultados Se estudiaron 223 niños, 51 presentaron hipertrigliceridemia, 27 hipercolesterolemia y 9 hiperglucemia. Comparando la eficiencia diagnóstica del ICE contra el IMC se encontró sensibilidad del 100 vs.56% para hiperglucemia, del 93% contra 70% para hipercolesterolemia y del 76% contra 59% para hipertrigliceridemia, especificidad, valor predictivo negativo, valor predictivo positivo, coeficiente de verosimilitud positivo, coeficiente de verosimilitud negativo y área bajo la curva fueron superiores para ICE. Conclusiones El ICE es un indicador más eficiente que el IMC para identificar riesgo metabólico en niños mexicanos de edad escolar.


Introduction Abdominal fat, particularly visceral, is associated with a high risk of metabolic complications. The waist-height ratio (WHtR) is used to assess abdominal fat in individuals of all ages. Objective To determine the ability of the waist-to-height ratio to detect metabolic risk in mexican schoolchildren. Patients and Method A study was conducted on children between 6 and 12 years. Obesity was diagnosed as a body mass index (BMI) ≥ 85th percentile, and an ICE ≥0.5 was considered abdominal obesity. Blood levels of glucose, cholesterol and triglycerides were measured. The sensitivity, specificity, positive predictive and negative value, area under curve, the positive likelihood ratio and negative likelihood ratio of the WHtR and BMI were calculated in order to identify metabolic alterations. WHtR and BMI were compared to determine which had the best diagnostic efficiency. Results Of the 223 children included in the study, 51 had hypertriglyceridaemia, 27 with hypercholesterolaemia, and 9 with hyperglycaemia. On comparing the diagnostic efficiency of WHtR with that of BMI, there was a sensitivity of 100% vs. 56% for hyperglycaemia, 93 vs. 70% for cholesterol, and 76 vs. 59% for hypertriglyceridaemia. The specificity, negative predictive value, positive predictive value, positive likelihood ratio, negative likelihood ratio, and area under curve were also higher for WHtR. Conclusions The WHtR is a more efficient indicator than BMI in identifying metabolic risk in mexican school-age.

ABSTRACT

The American College of Cardiology and the American Heart Association (ACC/AHA) 2013 joint guidelines for the treatment of hypercholesterolemia expand the indications for statin therapy. This study was performed to estimate the numbers of diabetic patients indicated for statin therapy according to the Third Adult Treatment Panel (ATP-III) of the National Cholesterol Education Program guidelines and the new ACC/AHA guidelines in Korea. We analyzed the data from the Korea National Health and Nutrition Examination Survey (KNHANES) 2010-2012. Patients with diabetes over 30 yr of age were analyzed by the two guidelines. Of the total 1,975 diabetic patients, only 377 (19.1%) were receiving drugs for dyslipidemia. Among 1,598 patients who had not taken any medications for dyslipidemia, 65.6% would be indicated for statin therapy according to the ATP-III guidelines. When we apply the new guidelines, 94.3% would be eligible for statin therapy. Among the total diabetic patients, the new guidelines, compared with the ATP-III guidelines, increase the number eligible for statin therapy from 53.1% to 76.2%. The new guidelines would increase the indication for statin therapy for most diabetic patients. At present, many diabetic patients do not receive appropriate statin therapy. Therefore efforts should be made to develop the Korean guidelines and to ensure that more diabetic patients receive appropriate statin therapy.
